I am awaiting payment of my $2,000. in unpaid winnings since about May 1. This withdrawal has been approved for payment since the beginning of May.Ironically, I had a much smaller win of about $538. which occurred after winning the $2,000., but this is the only cashout in that time period that they have actually paid. I am unwilling to deposit there again until I am paid my winnings in full. Surely, nothing else would make any sense.

Clearly you have devoted very little effort to examining this complaint.
Please provide complete records of deposits, bonus codes, balance reduced to zero events, withdrawals, and any alleged withdrawal cancellations (I didn't make any).
I *never* make a new deposit with new bonus code unless my balance shown on cashier page is zero dollars *and* zero cents. Sometimes it is difficult to lose the last few pennies, but I always find a way to do so.
When I have a withdrawable balance, I always make the withdrawal and wait for approval, with my balance showing $0.00 on the cashier page, before making a new deposit with new bonus code.
Hence, it is impossible that I had more than one bonus code in effect at any time.
You did not give me any cashout on this deposit. I received a cashout on an earlier deposit, but that withdrawal was approved *before* I made a new deposit with new bonus code. The fact that you took a long time to pay it is irrelevant. Nowhere do your terms say I have to wait until I actually have received the check before making a new deposit with bonus code.

That request was coming from a combination of 3 different promotions ( bonus codes).
Even that combination of promotions is forbidden as per our terms and conditions and your withdrawal should have been denied, we allowed you to obtain maximum cash out based on T&C.

Please at long last issue me a FedEx check for the $2,000. you owe me. I absolutely adhered to all of your requirements. I am a very experienced online player of very long duration and I always am absolutely sure to "zero out" my balance before any new deposits. Why not thoroughly review those transaction and you'll be able to very easily verify this "zeroing out" of my balance before every single new deposit. So it is absolutely unequivocal that I did NOT at any time mix bonuses!
I haven't played at Las Vegas USA since this non-payment for obvious reasons...but would love to resume doing so on a regular basis once I receive my cashout. And I'd like to point out that I was a VERY Heavy Depositor! I certainly deserve and have clearly earned payment of this perfectly legitimate cashout.
