Slots of Vegas Casino - Will not verify my documents, stalling payment

My withdrawal request was made for $1000 in January, It was approved for $650 and they took away $350 because I used code PRCPREFERRED bonus code. (350% PRC-Member only bonus with no playthrough requirement and no max cashout etc... My deposit was made using Players Reward Card. I supplied the PRC card number on the slots of vegas credit card verification form because that is what I used for the deposit. I contacted support and they did get my form but they said they need a credit card, so I sent in another form with the PRC card and my credit card as well. I have been a player with Slots of Vegas for over 5 years and have had my document verified previously. For the current withdrawal they asked for a credit card I have not had for years. I was able to get support to admit that was a mistake but now they are saying they have not received my documents over and over again. I have submitted my verification forms now 16 times and faxed them 3 times yet they continue to say they do not have them. I have also sent them in 3 different formats. (word, pdf, and jpg) with no success. With the forms I have included a copy of my ID, Credit Card, PRC card, and the utility bill. I wish they would simply say they have them and are reviewing them but they are repeatedly saying they never got them. I feel like I am being lied to.
