Can't get my account verified


I deposited 30 on a 400% bonus, did playthrough so decided to withdraw my winnings ($420 - $120 bonus). The casino said I cannot do any withdrawals without having a faxback form on file and to contact customer service. So I chatted with a customer service agent who said I would need to request my withdrawal again to receive an automatic reply email. I have tried twice and no faxback email link. Please help. I'm so sorry I'm only getting to this now--I was out of town all last week and am just now getting caught back up.

I've checked on this account and it appears there is a small documents issue, which I've just sent you an email about. As soon as I receive your reply, I'll be able to sort out your withdrawal. ;-)
