Captain Jack Casino - Can't get my account verified

I deposited 30 on a 400% bonus, did playthrough so decided to withdraw my winnings ($420 - $120 bonus). The casino said I cannot do any withdrawals without having a faxback form on file and to contact customer service. So I chatted with a customer service agent who said I would need to request my withdrawal again to receive an automatic reply email. I have tried twice and no faxback email link. Please help. AskGamblers Complaints Team have been informed that Captain Jack Casino representative will not be able to respond to ongoing complaints until Friday 13th of October, therefore the time frame is being extended with another 96 hours.

This complaint has been reopened as per Captain Jack Casino request and AskGamblers Complaints Team would like to give it one more try and help both parties involved into the dispute reaching to a satisfactory resolution.

I'm so sorry I'm only getting to this now--I was out of town all last week and am just now getting caught back up.

I've checked on this account and it appears there is a small documents issue, which I've just sent you an email about. As soon as I receive your reply, I'll be able to sort out your withdrawal. ;-)

Please let us know if you have cooperated the casino and sent the required paperwork. Keep in mind that in case you fail to respond within the given time AskGamblers Complaints Team will have no other option but to reject this complaint upon submitter's unwillingness to cooperate.
