Need permanent self exclusion

Hi, I would like help closing my account with Blind Luck as I am a gambling addict and have been trying for several months to get them to do this. All I get back is a form to fill out, and the maximum suspension period is 1 month. So I would need to send in a form every month forever to stay blocked with them. I don't think that's reasonable. Can you help? I filled out a form but wrote 'indefinite' instead of 1 month, but it was immediately rejected with the information that the maximum is 1 month. This is despite clear information that I am a gambling addict and need help. What can I do? I am desperate and don´t want to fall back into misery again.
