UPDATE: Today i recieved €170 in my bank account. I won the money just by playing a €10 no deposit bonus on the slot machine "Mystic Wolf". After the form was e-mailed to their customer support. The withdrawal was processed within a few days. I got the withdrawal accepted without having to deposit anything. I was actually suprised to see everything go so well. Hi, I didn't quite know where to put this.. but here goes: Today i managed to play through a bonus at Desert Nights Casino, i got the status "Cashout requirements are met" I have to make a deposit to claim the money i won, as well as supply documents to verify me. That is all ok, but one of the documents i was asked to provide is a form i have to fill out.. I am not quite sure what it implies. I have attached the form, which is a pdf file. What exactly am i signing when i fill out ths form and sign it to get my withdrawal? is it safe ? i would greatly appreciate it if someone could help me look this over. :-) Omolon faxback.pdf
