Plenty Jackpots is giving me the run around - I want my $500

I have been trying to collect my winnings from Plenty Jackpots for about 3 weeks.

The first time I submitted a request, I checked the option for bank wire. I entered all of my information and it just sat there for about a week. When I called in I was advised that I needed to send them a fax form with copies of my driver's license, credit card used to make deposits, and a utility bill in order to verify my identity. I was very uncomfortable sending copies of my credit card & driver's license! In any event, I did so that I could be paid. After 2 additional calls that got nowhere, I was told that the bank wire option was no longer available. So I was told to set up an account with My Paylinq.... I did that. I had to cancel my original payout request and go back and request another payout. My Paylinq is not an option in the drop down menu for payouts. So I called again and I was told to click on QuickTender. I have waited 4 days and went to check status & it hasn't changed. I just checked and QuickTender requests that you sign up with UseMyWallet which says "This Service Temporary Stopped." The other options are Click2Pay (temporarily unavailable), EWallet Express (does not operate in USA), and MoneyLine Wallet which only allows a max withdrawal of $100.

I won $500 playing Black Jack - FAIR & SQUARE AND I WANT MY MONEY. Can you PLEASE help me??
